Extension Services in Pharmacy at the University of Wisconsin-Madison School of Pharmacy is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. This program has been designated as an application-based program and is approved for 12 hours (1.2 CEUs) of pharmacy continuing education credit. Pharmacists will be required to complete a pre- and post-survey regarding educational objectives and a program evaluation. Statements of credit will be provided within 30 days of receipt of materials documenting participation of this program.

There are a limited number of rooms available at the preferred rate. The cut-off date for this rate is Monday, September 27, 2010. After this date or once the rooms are sold out, reservations are subject to standard hotel rates and availability.
Cancellation requests must be made in writing. All cancellations are subject to a $300 processing fee. No refunds will be made for cancellations after October 6, 2010.
